    Assignment Prompt & Instructions

    **Overview:**
As a nursing student, you'll be expected to connect what you learn in class to real-life patient care situations. This assignment gives you the chance to reflect on a beginner-level care scenario using what you've learned about nursing history, theory, ethics, cultural competence, caring, and your legal scope of practice.
    ##### **Scenario:**
You are beginning your clinical rotation at a long-term care facility. You’re assisting with the admission of Mr. Garcia, an elderly man who recently immigrated from Mexico. He speaks limited English, seems anxious, and expresses a strong desire to practice his spiritual traditions during care. You’re working under the supervision of a registered nurse (RN), who assigns you several tasks during the admission process.
    ##### **Your Task:**
Write a **750–1000 word reflection** using the scenario above. Use your learning from Weeks 1–3 to answer the following:
    ###### **Part 1: History, Theory, and Ethics**
    – How does nursing history or a nursing theory (such as Watson, Orem, or Roy) help guide your care for Mr. Garcia?
    – Identify one ethical principle (such as autonomy, beneficence, or justice) and explain how you would apply it in this situation.
    ###### **Part 2: Culture and Caring**
    – What culturally responsive and respectful strategies would you use when caring for Mr. Garcia?
    – Describe specific caring behaviors (like empathy, presence, or listening) that you would use, and how they connect to a caring framework like Watson’s Caritas.
    ###### **Part 3: Legal Scope and Professional Roles**
    – What is your role and scope of practice as a student nurse in this setting? What should be done by the RN?
    – What legal responsibilities must you follow in this scenario (such as confidentiality or informed consent)?
    ##### **Tips for Success:**
    – Use examples from class discussions, readings, or lectures.
    – You don’t need citations—just focus on showing your understanding.
    – Keep your response focused, reflective, and professional.

**A Concept Map Summary on Barriers to EHR-Based Medication Reconciliation Integration**

The incorporation of an Electronic Health Record (EHR)-based medication reconciliation system is a transformational opportunity that could be used to improve patient safety, minimize medication errors, and promote interdisciplinary collaboration. There are, however, several obstacles, as shown in the concept map below, to successful implementation in healthcare organizations:

    ![](https://www.sweetstudy.com/questions/bj-psda)

    1.
**Ethical Barriers:** Ethical issues arise due to the privacy of the patients and the misuse of health information. When adopting EHR systems, it is essential to follow the principles of confidentiality and data security because failure to do so may lead to a loss of patient trust and substandard care (Rittenberg et al., 2022). It is necessary to ensure patient information is distributed conscientiously across interdisciplinary teams without violating ethical standards.

    2.
**Legal Barriers:** Legal limitations include adherence to federal and institutional laws, including HIPAA and The Joint Commission laws. The discrepancies between the documentation and the misuse of the digital medication data may put organizations at risk of litigation.

    3.
**Financial Barriers:** The adoption of the EHR-based systems will require a significant investment in the software, hardware, and the training of the workforce (Ciudad-Gutierrez et al., 2025). Maintenance and system upgrades are also continuous costs. Budgets frequently require hospitals to postpone adoption even in cases where long-term returns, like gains in the reduction of errors and increased efficiency, have been demonstrated.

    4.
**Adoption Barriers:** Healthcare professionals can be accustomed to manual operations or overwhelmed by technology (Davis, 2023). It has been demonstrated that concept mapping helps facilitate professional learning and involvement based on the possibility of making abstract notions concrete and creating common ground between users.

    5.
**User-Related Barriers:** Issues of interest to the user include technical skills and workflow interference. The staff and the management might first have to cope with the workload as they learn the new system.

    **Introduction**
Nutrition is the cornerstone of health and healing. Nurses play a vital role in assessing patients’ nutritional status, identifying risk factors for dietary disorders, planning interventions, and evaluating outcomes. Nurses must integrate nutritional knowledge with clinical judgment to support optimal outcomes when caring for patients with chronic illness, hospitalized malnutrition, or those on therapeutic diets.
In this discussion, you’ll explore how to assess nutritional needs, identify at-risk clients, and propose evidence-based nursing interventions that promote comfort, health, and healing.
    ##### **???? Case Scenario: Complex Nutrition Risk**
**Patient:** Ms. Julie Green, a 71-year-old retired schoolteacher
**Medical History:** Congestive heart failure, type 2 diabetes, osteoarthritis
**Social History:** Lives alone, limited mobility, relies on frozen meals
    **Recent Labs and Vitals:**
    – Hemoglobin: 10.1 g/dL (low)
    – Albumin: 2.9 g/dL (low)
    – BMI: 18.3 (underweight)
    – Weight loss: 12 lbs in the last 2 months
    – Reports fatigue, poor appetite, and occasional dizziness
    – **Assessment:** Pale, frail appearance; reports skipping meals and limited grocery access.
    ##### **???? Discussion Prompt**
Choose **ONE** of the following categories and provide a detailed post:
    ###### **1. Nutritional Risk and Assessment**
    – What data would you collect to assess Ms. Green’s nutritional status?
    – Based on Dudek Ch. 15, what criteria suggest malnutrition, and how would you classify her risk level?
    – What role does the nurse play in screening and early intervention?
    ###### **2. Interventions to Promote Nutritional Health**
    – Propose three nursing interventions that address Ms. Green’s nutritional concerns (Taylor Ch. 37, Dudek Ch. 16–17).
    – How can these be adapted to her age, mobility limits, and chronic conditions?
    – How would you evaluate if your plan was successful?
    ###### **3. Alterations in Nutrition and Complications**
    – Which nutritional complications is Ms. Green at risk for, and why?
    – How do changes in protein, fluid, or calorie intake affect patients with chronic illness or aging?
    – What interdisciplinary referrals (e.g., dietitian, social services) might be needed?
    ##### **???? Instructions for Participation**
    – Post your initial discussion by Saturday at 11:59 PM CST.

    **## Riskmanagementinterviewinstructions.docx**

The purpose of this assignment is to gain real-world insight into how risk management programs operate within health care organizations.

Select a local health care organization where you can conduct an interview with an employee who is involved in risk management processes. This organization can be your current employer or a different health care facility in your community. Acute care, urgent care, large multi‐provider private medical clinics, assisted living facilities, and community/public health clinical facilities are all ideal options to complete the requirements of this assignment. Select an individual who can provide sufficient information regarding how their organization manages risk within its facility to answer the questions below.

In your interview, address the following:

    · Risk management strategies used in the organization's risk control program, along with specific examples.

    · How the facility's educational risk management program addresses key professional, legal, and ethical issues, such as prevention of negligence, malpractice litigation, and vicarious liability.

    · Policies the facility has implemented that address how to manage emergency triage in high‐risk areas of health care service delivery (e.g., narcotics inventories, declared pregnancy policies, blood-borne disease vector, etc.), and identify how these align with Occupational Safety and Health Administration (OSHA) standards.

    · Challenges the organization faces in managing and controlling high-risk health care (e.g., infectious diseases, nuclear medicine, abortion, class 4 narcotics/opioids, etc.), and include how cultural and religious beliefs affect the provision of patient care.

    · Strategies the facility utilizes to monitor, evaluate, and maintain compliance within its risk management program.

After conducting the interview, compose a 750‐1,000-word summary analysis of the interview that includes the prompts above, in conjunction with the interviewee's responses. In addition, include the following elements in your response:

    · An assessment of the organization's risk management program, including how it attends to high-risk health care and legal concerns.

    · Action steps you would take to improve one area of the organization's risk management program, along with your rationale for doing so.

Cite appropriate references as needed to support your statements and rationale.
