    Introducing Statistics for the DNP

A research problem statement identifies an area of need in which there is a gap in knowledge or practice and interest by a nurse researcher. Either from a hypothetical or real-world experience, nursing research problem statements help to focus the research purpose and guide the study of the problem.

In this discussion, you will identify a nursing problem to analyze the potential impact for positive change and explore tools used to examine it in the selected research.

Racial Disparities in Healthcare among Pregnant Women in the United States

    Tamifer Lewis

Department of Public Health, Monroe College, King Graduate School

    Literature Review

    Introduction to the Literature Review

Research suggests that racial disparities in healthcare among pregnant women persists in

the United States (Zhang et al. 2013). Due to this continuous occurrence, it is vital to examine

the factors that contribute to the adverse outcomes in maternal health. The literature review

contained only research articles about factors that impacted and influenced disparities in

pregnancy outcomes. Factors that were reviewed were socioeconomic status, public health

insurance, race/ethnicity, and poverty status. The literature review was conducted using EBSCO

Host and ProQuest databases from the Monroe College Library. The search terms used to

compile pertinent articles were racial disparities maternal health, adverse pregnancy outcomes,

    and maternal health outcomes.

    Review of the Literature

    Adverse Pregnancy Outcome Factors

Darling et al. (2021) conducted a study between 2001 and 2018 to examine the efficiency

of qualified interventions in preterm birth, small for gestational age, low birth weight, neonatal

death, cesarean deliveries, maternal care satisfaction, and coast effectiveness programs. A

systematic review was used to collect data from the United States, France, Spain, and the

Netherlands. The studies consisted of mostly non- Caucasian women from low-income

population ranging from 12 to 46 years of age and being between 20 to 32 weeks' gestation.

Interventional programs were implemented into three categories: group prenatal care, augmented

prenatal care, or a combination of both group and augmented prenatal care (Darling et al. 2021).

The researchers found that certain interventions, such as prenatal care and augmented care, or a

combination of both, may decrease adverse outcomes in small-for-gestational-age and preterm

    3

birth, and could aid in increasing maternal care satisfaction. Interventions that worked on

enhancing coordination of care were found to result in providing more effective cost savings.

The researchers also found disparities in the quality of access to care in the vulnerable

population. There was insufficient evidence of suitable quality to confirm that the interventions

were successful at enhancing clinical outcomes in prenatal care for at risk populations (Darling et

    al. 2021).

Similar observations were made in a study conducted by Nichols and Cohen (2020),

between 2006 and 2018 to examine the methods used to improve the results of maternal

mortality in California. The study was conducted using a scoping review to evaluate research on

women and maternal health in the United States. The researchers used information from the US

Maternal Fetal Medicine Network to measure the percentage of studies where pregnant women,

women, and children were the main focus. The researchers also reviewed documentation on

healthcare policies and practices from California’s public health department, healthcare

foundation, and Maternal Quality Care Collaborative. Nichols and Cohen (2020) found that

although the health of fetus and children could be adversely affected by the health of the mother,

the majority of maternal programs in the United States places emphasis on the child. The

researchers also found four areas of concern in women health experiences, both in pre and

postnatal care. The problem areas entailed inadequate investment in women's health, inefficient

quality of care and avoidable caesarean delivers, expanding disparities in minority women and

women living in rural areas, and contradictory collection and distribution of data (Nichols and

    Cohen, 2020).

    4

    Approaches to Improving Pregnancy Outcomes

In contrast to the preceding studies, Zhang et al. (2013) conducted a study between 2005

and 2007 to calculate the excessive rate of unfavorable outcomes in pregnancy within racial and

ethnic groups. The study also aimed to measure the possibility of Medicaid savings that are

linked to paid maternal care claims resulting from the inequalities that contribute to unfavorable

maternal outcomes. A cross-sectional study using Medicaid Analytic eXtract (MAX) data was

used to gather pregnancy outcome information from inpatient hospitals from 14 states (Florida,

Alabama, Arkansas, North Carolina, Georgia, Louisiana, Kentucky, Mississippi, Maryland,

Missouri, Tennessee, South Carolina, Virginia, and Texas). The study consisted of a little over 2

million patients who were insured with Medicaid and had a delivery code of maternal delivery

stay. Zhang et al. (2013) found that, with the exception of gestational diabetes, African American

women showed the worst outcomes out of all unfavorable pregnancy outcomes. These disparities

are postulated as being multi-factorial, having causes stemming from complicated experiences

with racism, poverty, and complex healthcare interactions. It was also found that women covered

under Medicaid health insurance were more likely to have consistency in care from prenatal care

through delivery compared to their counterparts. However, due to participation in Medicaid

programs being influenced by reimbursement rates, some providers may choose to stop

accepting Medicaid patients because of reimbursement delays and low payment rates, which

could contribute to negative birth outcomes (Zhang et al. 2013).

    Analysis of the Literature

In the United States, the persistence of maternal mortality continues to be a problem area

in public health. The contributing factors that impact pregnancy outcomes persist in burdening

the U.S., leading to poor healthcare quality, and increasing health disparities. The studies used in

    5

this literature review each used a different form of research methodology to collect data,

including systematic and scoping reviews and cross-sectional studies. Similarly, Darling et al.

(2021), Nichols and Cohen (2020), and Zhang et al. (2013) have emphasized the correlation

between race/ethnicity and financial status playing a part in influencing quality of care, access of

care, and pregnancy outcomes in pregnant minority women. To mitigate the disparities in

maternal health Darling et al. (2021) and Zhang et al. (2013) suggested that interventions should

be inspected and geared towards determining and eradicating the racial and ethnic disparities that

affect pregnancy-related outcomes. Whereas Nichols and Cohen (2020) suggested focusing on

exploring the distinctive experiences of particular at-risk subgroups of women, such as women in

prison, who are of childbearing age, and the pregnant women who are less likely to pursue

prenatal care, such as undocumented women.

    Review of the Literature

    Adverse Pregnancy Outcome Factors

Darling et al. (2021) conducted a study between 2001 and 2018 to examine the efficiency

    Response 1 Part 1:

A multisensory experience occurs when someone applies multiple senses such as sight, sound, touch, taste, and smell to a presentation. Using this method can be the difference between your audience simply hearing you, and your audience actually learning and taking in what you're presenting. According to Gallo, using multisensory experience enhances learning. due to it allowing the brain to build two models, visual and verbal. This makes learning much more effective. This method goes beyond just presentations. Using this method can also be very effective when it comes to being a good leader. A multi-sensory approach can significantly enhance team building, communication strategies, and overall a better stimulating work environment. According to the article "Multi-sensory Approach to Managing Information Overload at Work" by Cauvery Bhalla, using this method helps ensure your employees retain the information that is given to them. Using this method can also prove to be more fun and creative. "Eighty-one percent of employees at companies ranked as "great" described their office environments as fun." Some examples of how to use this method in an office environment as a manager are using chart papers, colorful post-its, playing music, having smell-good plug-ins, field trips, role play, and presentations with pictures as said by Gallo. These are all just a few reasons why using the multisensory method is beneficial and important in today's day and age and how you can implement it.

    Part 2:

The first thing I noticed in this video was the use of the apples and bananas to help visualize the math problem that Roberto was working on, even down to eating the bananas. Another thing they used was sound by reading the problem aloud. These two senses being stimulated will help the brain process the problem a lot easier.

    Response 2

For this discussion, I focused on the purpose of the multisensory experience in education. Multisensory instruction involves children using more than one sense at a time. When you incorporate more than one way to make connections and learn concepts it leads to better understanding. The five senses are taste, smell, touch, sight, hearing, and movement. Some benefits of multisensory experience in education are better memory of the skill, improved concentration, support for different learning styles, encouraged active learning, improved cognitive development and critical thinking skills, boost to student's confidence, and helps to make it easier for children with learning disabilities. Multisensory instruction is not only used in reading but also in math and science. For example, a math instructor may use interlocking cubes or shape blocks to create a visual for students. As an instructor you can you images and video to convey your message.

In the video, the mom and son used exaggerated gestures to display their frustration with the principal such as hand movements and raised voices. Mr. Sanford used his tone and descriptive language to express his concern for Roberto. He focused on creating an emotional connection with the principal to persuade him to tutor the student. In the end of the meeting, the principal stood up and shook Mr. Sanford's hand as a sign of friendship and trust. At the end of the video, Mr. Sanford and Roberto are shown working on math problems through the use of props to create a visual of the problem.

Kohl's fired 3 employees for violating store's policy
    BY MEREDITH BONNY
    TIMES-DISPATCH STAFF WRITER
    Friday, April 15, 2005

Rikki Groves wasn't the only employee fired from the Kohl's
department store in Mechanicsville and asked to pay
restitution for reusing coupons and passing the savings on to
    customers.

Rebecca Hagen, 20, of Sandston and Katie Kombacher, 21, of
Mechanicsville said they were also interrogated,
photographed and asked to sign documents barring them
from returning to the store or shopping there after they were
    told they misused coupons.

"I was pretty mad about the situation," Hagen said. Kombacher said her firing was "a
big ordeal." She said store officials "made me walk through the store while I was
    crying."

All three women are looking for new jobs.

Earlier this week, The Times-Dispatch ran a story reporting what had happened to
    Groves.

Dozens of people responded. The newspaper received more than 50 e-mails and
about 40 phone calls, complaining about the way Groves was treated. Hagen and
Kombacher contacted The Times-Dispatch after the story ran.

    Many readers who
    contacted The Times-
    Dispatch said they planned
    to call the store's corporate
    office to say they would
    never shop at a Kohl's
    again. Others said they cut
    up their Kohl's credit cards.

    A few questioned why the
    newspaper ran the story,
    calling the incident minor.

    When they were dismissed
    from the store, all three
    women were told they

would have to repay the store for discounts taken with the extra use of the coupons.
Hagen said store officials "told me if I did not pay, I would have been arrested for
    embezzlement."

    AUDIO: Rikki Groves,
    former Kohl's employee

    Kohl's worker fired over
    coupons

    Kohl's fired 3 employees for
    violating store's policy

Groves said she was told she owed the store $1,000. The evening before the first
story went to press, Lori Sansoucie, a spokeswoman at Kohl's headquarters in
Menomonee Falls, Wis., said that while Groves violated company policy, the store
would not require her to pay back the $1,000.

At that time, Groves had already paid the store $100 and was scheduled to pay $150
each month for the next six months.

She has since received a $100 check in the mail from the store.

Hagen, who had been asked to pay $214, and Kombacher, who had been asked to
pay $600, said they have also gotten their money back.

Sansoucie yesterday confirmed that three employees had been fired for the same
"policy infringement," but she would not comment on the way the women said they
    were treated.

Groves, Hagen and Kombacher all said that they were originally told by a security
employee there that they could be arrested if they did not pay the money back.
However, a Henrico County police spokesman told The Times-Dispatch that to his
knowledge the matter in question was not a crime and rather an internal store-policy
    matter.

The controversy stemmed from a recent "friends and family sale" at the store.

Groves said Kohl's employees received 15-percent-off coupons in the mail — one for
their use and 10 for family and friends.

All three former employees said they had extra coupons and in some cases were
reusing them at their registers. They said they were not told what they were doing
was wrong until it was too late.

"I don't think I was treated fairly," Kombacher said.

Looking back, Groves said that while she is glad she won't have to pay back the
$1,000, she's still angry at how Kohl's handled the issue.

"I never received an apology," Groves said. "But at least now I know for sure now
    that they were wrong."

Kohl's fired 3 employees for violating store's policy
    BY MEREDITH BONNY
    TIMES-DISPATCH STAFF WRITER
    Friday, April 15, 2005

Rikki Groves wasn't the only employee fired from the Kohl's
department store in Mechanicsville and asked to pay
restitution for reusing coupons and passing the savings on to
    customers.

Rebecca Hagen, 20, of Sandston and Katie Kombacher, 21, of
Mechanicsville said they were also interrogated,
photographed and asked to sign documents barring them
from returning to the store or shopping there after they were
    told they misused coupons.

"I was pretty mad about the situation," Hagen said. Kombacher said her firing was "a
big ordeal." She said store officials "made me walk through the store while I was
    crying."

All three women are looking for new jobs.

Earlier this week, The Times-Dispatch ran a story reporting what had happened to
    Groves.

Dozens of people responded. The newspaper received more than 50 e-mails and
about 40 phone calls, complaining about the way Groves was treated. Hagen and
Kombacher contacted The Times-Dispatch after the story ran.

    Many readers who
    contacted The Times-
    Dispatch said they planned
    to call the store's corporate
    office to say they would
    never shop at a Kohl's
    again. Others said they cut
    up their Kohl's credit cards.

    A few questioned why the
    newspaper ran the story,
    calling the incident minor.

    When they were dismissed
    from the store, all three
    women were told they

would have to repay the store for discounts taken with the extra use of the coupons.
Hagen said store officials "told me if I did not pay, I would have been arrested for
    embezzlement."
